Plan Features

Most vision plans cover the cost of one exam per calendar year and provide additional coverage to help offset the cost of lenses and frames or contacts. Like health plans, there are several types of vision plans. Unsure of which plan is right for you? INDEMNITY PLAN

You may visit any provider you choose, but you will be required to cover the cost of care in full at the time of service.

PPO

When you visit an in-network provider, you will save more. In-network providers have agreed to negotiated rates, meaning you will pay less when you seek care.

HMO

You are typically required to designate an in-network primary care provider and receive care only from them for vision services. Out-of-network coverage is not available.
